IdentityServer4 - Token refresh doesn't appear to work first time

Question

I have an ASP.NET Core MVC app that is using IdentityServer 4 as an identity provider. To aid debugging, I\'ve set the IdentityServer cookie to expire after 5 minutes and I\'ve
