Playing videos using HTML5 Tag

I am creating a website where videos need to be streamed. The content creator uploads .mov file to the server. Upon uploading, I convert to the following formats using FFMPE

It is working. The edits in the original post is the final! Hope it helps anyone else who might have similar problem. Just check if you have set any authentication in your server if it is not working first. :)
